1. REQUIRED: Complete an ONLINE HEALTH RISK ASSESSMENT.
– Sign into Member Online Services at www.HorizonBlue.com;
– Select “Wellness”. Under “NJWELL”, select “Step 1, Health Assessment”;
– Under “Healthy Living”, select “Health Assessment Tool”;
– Answer all questions, press “Finalize”; answer remaining questions and press “FINISH”. When completed, you’ll see a new Health Score and the points you’ve earned.
2. REQUIRED: Complete a BIOMETRIC HEALTH SCREENING with your physician.
– Sign into Member Online Services at www.HorizonBlue.com;
– Select “Wellness”. Under “NJWELL”, select “Step 2, Biometric Health Screening” to download the Physician Results Form at My.QuestForHealth.com. Additional instructions are on the form.
** IMPORTANT: If you are enrolled in the Direct Primary Care Medical Home (DPCMH) program with R-Health or Paladina Health, your DPCMH doctor can perform the Biometric Health Screening for you and a form will not be necessary. Learn more here.
3. ADDITIONAL ACTIVITIES, if needed to achieve 800 points.
